As a new participant in the Sound Health & Wellness Trust, you are most likely enrolled in the Sound (formerly Plan B) Plan and are eligible to participate in Group Health Options plan (or PPO Out-of-Area plan if you live outside of a Group Health coverage area) until you have completed 35 months of employment with a contributing employer. Here are some of the forms and information you need to enroll in the Sound Plan.
Some new participants may be eligible to enroll in the SoundPlus (formerly Plan A) Plan. Here are the forms and information you need to enroll in SoundPlus Plan.
