Each year Sound Health & Wellness Trust participants and covered spouses or same-sex domestic partners (SSDP) have the opportunity to take their PHA. Taking the PHA is one step towards avoiding a higher deductible and to learn more about your health status. The PHA is offered for a limited time every year – you will receive notification from the Trust Office when it is available.
The LiveWell Personal Health Assessment is a quick and simple annual questionnaire that gives you an immediate snapshot of your current health and health risks, and gives you a personal plan for healthy living. Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with the Trust or your employer without your permission. This is a tool for you to use in talking with your doctor and making healthy lifestyle changes.
The LiveWell Personal Health Assessment is designed to help you review your ongoing health needs each year and understand them more thoroughly, so you can take full advantage of all the great services available to you. Each year, there will be a limited period of time when you (and your eligible spouse or same sex domestic partner if you have family coverage) can take the Personal Health Assessment.

Confidentiality is paramount
All benefits and LiveWell wellness programs are provided and managed by independent service providers, contracted by the Trust. The information on your health status and health conditions, which comes from your responses to the Personal Health Assessment, your medical and prescription drug claims, and information you may provide during a discussion with representatives of the Trust’s service providers is only used by these independent service providers to offer you programs that may be of interest to you or help you in your health and wellness goals.
Please know that information on your health status and health conditions is used only between the independent service providers. You may be contacted by phone, email, or U.S. mail to let you know about and ask you to participate in a number of voluntary LiveWell programs that are relevant to your situation and health status. For example, you may be contacted about a tobacco cessation program, a health-coaching program, or a program to help you manage a chronic medical condition.
By law, your health information cannot be used to determine or deny health care coverage. Your health information is completely confidential, protected by federal law, and cannot be shared with the Trustees, your union or your employer without your permission.
The first step is fast and easy
The assessment begins with a quick, confidential questionnaire that you can fill out online, or by requesting a paper copy to be mailed to your home. The questionnaire prompts you to stop, think, and respond with honest answers about everyday factors such as nutrition, stress, physical activity, weight management, tobacco and alcohol use, and much more.
Useful information, instantly
The process takes about 15 minutes, and the moment it’s complete, the online system performs an instant analysis and presents you with your very own profile that not only rates your current health status, but projects your potential future health problems as well.
Best of all, your PHA also provides you with a personal plan for healthy living—one designed especially for you. It explains the relationship between your behavior and your health, and outlines the steps you can take to improve your health.
More health options
When you submit your personal health assessment, you may also qualify for LiveWell Health Coaching or Condition Management services, designed to deliver high-quality, personalized support for better health. These programs are offered at no additional cost to you, and are a great way to get extra support.

Remember, information on your health status and health conditions is shared only with the independent service providers that manage the LiveWell programs and is not shared with the Trust or your employer.
