MySoundHealth gives you access to MyLiveWell, as well as your personal health benefits information, eligibility status, work history, claim history and pension information (if applicable).

Each year, the Sound Health & Wellness Trust establishes a LiveWell HRA for eligible plan participants, and provides funding up to $500 for employee only coverage and up to $1,000 for family coverage for those participants who complete a variety of health-related actions. The funds in your HRA are used to cover the first eligible medical expenses covered by the plan in each calendar year. This can include your doctor office visits, lab tests, or other eligible, covered medical expenses.
In order to receive HRA funding and avoid a higher deductible, you and your eligible spouse or same sex domestic partner (SSDP) can complete health related actions each year. You will receive notification from the Trust regarding the necessary steps required each year and by when, to receive your HRA funding and avoid a higher deductible. If you do not complete the necessary steps each year, you will not receive full HRA funding, which means your deductible will be higher. The maximum annual HRA funding is $500 for employee only coverage and $1,000 for family coverage.
For your 2013 HRA funding you will need to complete all of the following steps to receive $350 in funding: complete your Personal Health Assessment, update or confirm your contact information, and maintain or select your in-network primary care physician (PCP) online.
For the remaining $150 in 2013 HRA funding, you and your eligible spouse/SSDP have many health related actions to choose from throughout 2012. The actions, if completed by November 30, 2012, are worth $50, $100 or $150 in HRA funding.

Learn More About the PHAWhat kinds of medical expenses will my HRA cover? If you have enough funds in your HRA, the following are examples of what can be covered:
- Doctor office visits
- Lab tests
- X-rays
- Emergency room co-pays
- Coinsurance (SoundPlus Plan participants are eligible for in-and-out-of-network coinsurance benefits. Sound Health Plan participants only receive in-network coinsurance benefits).
Remember: Eligible in-network preventive care like wellness visits and mammograms are already covered in full, with no deductibles, coinsurance or co-pays, so your HRA will not be used for these types of services.
Earning your LiveWell HRA funding is simple.
If you (and your enrolled spouse or same sex domestic partner) complete all necessary steps in the time period specified, you will receive your full funding – the maximum annual HRA funding is $500 for employee only coverage and $1,000 for family coverage.
You do not need to submit any claims to use your LiveWell HRA. Everything will be handled behind-the-scenes on your behalf. Your online account will show your activity as well as your balance.
This year’s HRA funds can be used next year.
Unused funds in a calendar year are rolled over into the next year depending on how long you were eligible and as long as you maintain your benefit eligibility status.
