LiveWell Nurse Line Plus
Call (877) 362-9969 24 hours a day, 7 days a week. Call anytime and knowledgeable registered nurses will confidentially help you find the information you need to make informed decisions. Unlike traditional nurse lines, nurses are available to answer your health-related questions:

  • Resolve health concerns
  • Navigate the healthcare system
  • Learn how to care for yourself at home
  • Get guidance about medical procedures
  • Find a healthcare provider
  • Receive healthy living resources

LiveWell Personal Health Assessment (PHA)
Each year, there will be a limited period of time when you or your eligible spouse can take a confidential Personal Health Assessment and receive an incentive. You can take the PHA at other times of the year as well, but without the incentive.

The PHA will give you snapshot of your current health status along with information and recommendations for how to get and stay healthy. Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with anyone (including the Trust or your employer) without your permission.

LiveWell Health Coaching
If, through your Personal Health Assessment, you are identified as possibly needing help in a specific area, you will be offered the chance to take advantage of personalized health coaching at no additional cost. The LiveWell Health Coaching program is a phone-based health education program designed to help you set and meet goals to improve your health and well-being. Your health coach will provide you with educational materials, guidance, and the support you need to begin to make healthy lifestyle changes.

LiveWell Prevention @ 100%
An ounce of prevention is worth a pound of cure! In-network preventive care is now completely covered - with no deductibles, coinsurance or co-pays.

LiveWell Condition Management
Custom-tailored services are available for participants with chronic conditions such as diabetes, heart disease, asthma and other pulmonary diseases. Program participants can work one-on-one with a personal nurse advocate to improve both their health and their quality of life.

LiveWell Health Reimbursement Arrangement (HRA)
Each year, each employee will receive a Trust-funded Health Reimbursement Arrangement—$500 for those with employee-only coverage and $1,000 for those with family coverage. (These amounts may be lower if you are a participant for less than the full year.) This money will be used to cover medical expenses for such things as your doctor office visits, lab tests, or other eligible, covered medical expenses.

Frequently asked questions about the LiveWell HRA

LiveWell Tier 0 Prescriptions
A new “Tier 0” has been added to the Trust’s therapeutically based prescription plan. For the highly cost-effective medications under Tier 0, there is $0 co-pay for participants!

Prescriptions under Tier 0 will initially include cholesterol lowering medications (Simvastatin), proton pump inhibitors (Prilosec OTC, with physician prescription), and non-sedating antihistamines (generic Claritin, with physician prescription).

LiveWell Online
Log on to the Secure Portal, above. From this secure site, you can access LiveWell Online, our special health and wellness site where you can find health information, tools and resources to help you creative positive lifestyle changes. What you’ll find:

  • The Family Center provides information on self-care topics like immunizations, men’s and women’s health issues, and more.
  • Look It Up serves as a drug and nutrition guide. What If gives you the opportunity to take a PHA again to see how your lifestyle changes have benefited your health.
  • Health Tools will help you create realistic goals and set reminders for preventive screenings, appointments and exercise sessions.
  • Take Action offers a collection of online guides for various health conditions.
