LiveWell is a revolutionary benefit program—offered at no additional cost—that gives Sound Health & Wellness Trust participants and their eligible dependents direct access to the medical expertise, information and personalized support they need to make better health decisions and enjoy healthier, happier lives.

LiveWell services are provided by Group Health Options and other independent service providers, not by the Trust. Any information you share with any one of the LiveWell programs is completely confidential, protected by federal law, and cannot be shared with the Trust or your employer without your permission.

You may be contacted by phone or mail to participate in a number of voluntary LiveWell programs that are relevant to your situation and health status. For example, you may be contacted about a quit smoking program, a lifestyle coaching program to work on a particular area of concern such as exercise or nutrition, or a program to help manage a chronic condition such as diabetes. Information about your health status is gathered from the answers on your Health Profile as well as your medical claims. It is used to make you aware of free services you can take advantage of to improve your health and wellness.

Please be aware that information on your health status and health conditions is shared only with independent service providers and is not shared with the Trust or your employer. By law, this information cannot be used to determine or deny health care coverage.

• • Health Profile / PHA—An online questionnaire that gives you a snapshot of your current health and health risks, and gives you a personal plan for healthy living.

  • Consulting Nurse Helpline—A free 24/7 nurse line with live registered nurses.

  • Weight Watchers®—A nationally renown program that teaches you how to lose weight and to keep it off. Trust participants can enroll at a special rate and even get up to nine months at no cost if active participation requirements are met.
