Each year Sound Health & Wellness Trust participants and covered spouses or same-sex domestic partners, 18 years and over, can take their Health Profile/Personal Health Assessment and receive incentives. This year it was a free duffel bag, your choice of a $40 check or $100 HRA contribution and a chance to win a Nintendo Wii! While you can still take your Health Profile, this year’s incentive period ended May 30, 2010. Plan on taking your Health Profile early next year to track your progress and earn your reward. The Health Profile is designed to help you review your ongoing health needs each year and understand them more thoroughly, so you can take full advantage of all the great LiveWell services available to you.
The Health Profile is an online questionnaire that gives you an immediate snapshot of your current health and health risks, and gives you a personal plan for healthy living. Your Health Profile will even be integrated into your electronic medical record if you get your care at Group Health medical centers.
Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with the Trust or your employer without your permission. This is a tool for you to use in talking with your doctor and making healthy lifestyle changes.
Have your Group Health Options ID card handy—you’ll need your member ID to take the Health Profile. If you have not yet registered for MyGroupHealth, please follow the instructions on the web site. You must register for enhanced services to access the Health Profile. The verification process requires 3–5 days to receive a PIN code through the US Post Office.

Confidentiality is paramount
Based on your answers to the questions in the Health Profile, you may be contacted by phone or mail to participate in a number of other voluntary LiveWell programs that are relevant to your situation and health status. For example, you may be contacted about a quit smoking program, a lifestyle coaching program to work on a particular area of concern such as exercise or nutrition, or a program to help manage a chronic condition such as diabetes.
Please be aware that personal information on your health status and health conditions is shared only with the independent service providers that manage the LiveWell programs and is not shared with the Trust or your employer. By law, this information cannot be used to determine or deny health care coverage.
The first step is fast and easy
The assessment begins with a quick, confidential questionnaire that you can fill out online. The questionnaire prompts you to stop, think, and respond with honest answers about everyday factors such as nutrition, stress, physical activity, weight management, tobacco and alcohol use, and much more.
Useful information, instantly
The process takes about 15 minutes, and the moment it’s complete, the online system performs an instant analysis and presents you with your very own profile that not only rates your current health status, but projects your potential future health problems as well.
Best of all, your Health Profile also provides you with a personal plan for healthy living—one designed especially for you. It explains the relationship between your behavior and your health, and outlines the steps you can take to improve your health.
More health options
When you submit your Health Profile / Personal Health Assessment, you may also qualify for Lifestyle Coaching or Condition Management services, designed to deliver high-quality, personalized support for better health. These programs are offered at no additional cost to you, and are a great way to get extra support.
