MySoundHealth for Group Health Options gives you access to your personal health benefits information, eligibility status, work history and pension information (if applicable). You can also access LiveWell NutriSum and your HRA information.

Custom-tailored services are available for participants who have been diagnosed with chronic conditions such as diabetes, congestive heart failure and coronary artery disease. Program participants can work one-on-one with a personal nurse advocate to improve both their health and their quality of life.

If you are eligible, you can receive $30 a month into your HRA account, up to a total of $270 per year. You will receive a call inviting you to participate in this program, or you can call to see if you qualify.

  • Your own, personal nurse advocate.

    Condition Management participants work one-on-one with a personal nurse advocate—a registered nurse who has extensive experience with your condition.

    Your nurse will consult with you over the phone to assess your health, review your care, and discuss any other medical concerns you may have. Your nurse advocate can help you make the most of every visit to your doctor, and coordinate many other healthcare services available to you as well. You’ll get continuing telephone assistance, free educational materials to help understand your condition better, and ongoing support for any healthy behavior changes you decide to make.

    A more active role in your own health.

    The most important benefit of Condition Management is the power it gives you to play a more active part in your own health. In addition to providing the information, coordination, and support you need to manage your condition more effectively, your nurse advocate will also help you find the focus and motivation to harness the enormous power of your own desire to be well. And that can make all the difference.

    From practical changes you can make to improve your health to greater confidence managing your medical condition, the plan you design with your personal nurse advocate will address your needs as a whole person, not just your disease or condition.

    You can be sure that any information you share with your nurse advocate is protected by federal law and cannot be revealed to Trustees, your union or your employer without your permission. Of course, Condition Management programs are intended as a supplement to, not a substitute for, your doctor’s care. For diagnosis and treatment, always consult your physician.
